Community Care, Inc., ("CCI"), a Tennessee Corporation, was organized in February 1998. The company's purpose is to acquire, develop, and manage freestanding ambulatory surgery centers in partnership with physicians and hospitals. Using a custom design approach, diagnostic and rehabilitation services can be added to meet the community's demand for comprehensive outpatient healthcare services.

In April of 1998, CCI conducted a successful Private Offering that raised $7.5 million in initial capital. The company has since created a credit facility for acquisitions and other capital needs required to fund development projects.
